Requirements for Certification

1. Duly Accomplished CECO Application Form (Annex A)
2. Endorsement Letter from the head of the Type 1 DE
3. Certified True Copy of Academic Credentials
4. Sworn Statement Annex B
5. Proof of payment of One Thousand Pesos (PHP 1,000.00) for the Application Fee for Certification
